Job Summary

The Palliative Care Nurse Practitioner's primary responsibility, in collaboration with the supervising physician, is to provide system-wide consultation in palliative care, pain, symptom management, and supportive care to meet the needs of the patient and family. Combines clinical practice with education, research, consultation, and leadership to oversee the Palliative Care Service. Responsibilities include performing history and physical examination, ordering and interpreting radiological and laboratory tests, ordering and prescribing medications, documenting patient care services within the EMR and departmental records, and performing procedures within scope or practice demonstrated through competency.

Job Specific Duties

  • Facilitates patient/family encounters and assists the healthcare team to clarify patient and family goals and individualized plan of care. 
  • Serves as an expert resource and consultant to physicians, clinicians, and other units/departments as necessary in the area of specialization. 
  • Consults with palliative care physician, informs primary physician of recommended services, and collaborates with other physicians to contribute to the patient’s individualized plan of care. 
  • Provides system-wide training, clinical practice, as well as physician and staff development pertaining to palliative care. 
  • Prepares and maintains accurate patient records, charts and documents to support the medical practice, and reimbursement for services provided. 
  • Presents results, trends, or conclusions from analysis of performance improvement initiatives. 
  • Utilizes data to develop process improvement plans as indicated to improve palliative care program outcomes.
  • Monitors and evaluates palliative care standards; makes process improvement recommendations. 
  • Works closely with the Palliative Care Medical Director to develop clinical practice guidelines & competency, utilize quality improvement findings to develop process improvement initiatives. 
  • Works closely with Leadership to assure that clinical protocols, case management, and quality improvement initiatives are implemented according to hospital policies. 
  • Participates in developing, educating, and integrating system-wide palliative care policies and documentation tools. 
  • Attends seminars/conferences as necessary to maintain knowledge base regarding palliative care. 
  • Communicates effectively with physicians, clinicians, patients, and families both verbally an in writing. 
  • Maintains membership in a national specialty organization.

Qualifications

Minimum Job Requirements

  • Master's Degree in Nursing 
  • Valid RN and APRN License - must be in active and in good standing throughout employment
  • Pediatric Nurse Practitioner (PNP) certification; or 5 years of PICU NP experience and PNCB, AANP, or ANCC Certification. Certification must be active and in good standing throughout employment.
  • American Heart Association BLS - maintain active and in good standing throughout employment
  • American Heart Association PALS - maintain active and in good standing throughout employment
  • 1 year of pediatric Nurse Practitioner experience

Knowledge, Skills, and Abilities

  • Strong interpersonal, communication, collaboration, and conflict resolution skills.
  • Ability to work independently and collaboratively with other disciplines using discretion and independent judgment.
  • Able to relate cooperatively and constructively with patients, families and co-workers.
  • Able to maintain confidentiality of sensitive information.
  • Ability to interpret, adapt and react calmly under stressful conditions
  • Bilingual in English and Spanish preferred.
